Disease Description: A morbid condition in which the urine contains chyle or fatty matter, giving it a milky appearance. Chyluria is commonly due to obstruction anywhere between the intestinal lymphatics and the thoracic duct, which causes rupture of renal lymphatics into the renal tubules. It may occur as a result of obstruction of the retroperitoneal lymphatics in bancroftian filariasis. This very rare and interesting condition in temperate regions is due to lymphatic connection with the urinary passage. In the tropics it is due to parasitic origin and caused by rupture of the renal lymph vessels, which become obstructed and weakened by the filaria sanguinis hominis. Chyluria is the excretion of milky urine due to admixture of chyle with urine. A minority of the affected subjects complain of gross haematuria. The condition is due to the blockage of the retro-peritoneal lymph nodes below the cisterna chyli, with consequent reflux and flow of the intestinal lymph directly into the renal lymphatics which may rupture and permit flow of chyle into the urinary tract. Microfilaraemia may or may not be present in these patients. The urinary sediment may contain microfilaria and red cells but seldom leucocytes. |
